Collection Requirements
This test should be ordered at Doctor Offices ONLY. Covenant HealthCare Laboratory does not perform or send these tests out.
Cologuard is available by prescription only and must be ordered by your doctor. Most commonly, you would discuss screening with your primary care physician, nurse practitioner, or other healthcare provider as part of a regular wellness appointment. Cologuard cannot be purchased over the counter.
If your doctor has given you a prescription for Cologuard, please call Cologaurd’s Customer Support Center at 1-844-870-8870. Their support specialists will make sure your prescription includes everything needed to complete your order.
A collection kit is sent directly to your preferred mailing address. You can store your kit until you are ready to use it, just store at room temperature in a cool, dry place. Keep way from heat and direct sunlight and out of reach of children.
Transport
The Collection Kit (with the sample inside) should be mailed back to Exact Sciences Laboratories via UPS, using the prepaid and pre-addressed return label that was provide. You can drop off the kit at any UPS store or shipping location, or you can call 1-800-PICKUPS to schedule a pick-up at your home.
Please note that the sample must be shipped within 24 hours of collection, and received by Exact Sciences Laboratories within 72 hours. Exact Sciences Laboratories recommends shipping it Sunday through Thursday for this reason.

Additional Information
Cologuard is not a replacement for diagnostic colonoscopy or surveillance colonoscopy in high risk individuals.
